Walkingtogether
The Walkingtogether programme delivers a comprehensive
programme of free health led walks. The
Walkingtogether scheme provides a platform for individuals to
improve their quality of life by increasing physical activity
levels and promoting wellbeing. Risk factors for obesity include
vascular disease, cancer, type 2 diabetes, high blood pressure and
high cholesterol levels, which relate to lifestyle issues such as
low levels of physical activity, poor diet, stress and poor
mobility.
Walking is an excellent activity to
participate in, it’s a safe activity which helps to improve
overall health.
Looking for simple, easy steps towards a
healthier lifestyle? Why not take part in Healthy Walks taking
place across various parks in Hackney? Suitable for all ages,
especially those who are beginners to exercise feel unfit or are
over 50. The walks take between 30 – 60 minutes and are led by a
qualified Health Walks Leader & First Aider.

Healthwise
Healthwise is a Physical Activity Referral Scheme (PARS), where
Health Professionals can refer patients to a low cost physical
activity programme. It is designed for individuals with existing
health conditions, such as CHD and Asthma, as well as those at risk
of developing health conditions, including CHD, diabetes, obesity
and depression.
Healthwise is designed for those patients who are not currently
active, but who would benefit from physical activity. We recommend
that you explore thoroughly with each patient whether they are
ready to start an exercise programme, and ask that you read the
inclusion criteria carefully before referring a patient. This is
not a long term solution for those requiring one-to-one
attention.

Healthy Lifestyles
The Healthy Lifestyles Scheme is an innovative project working
with 7-13 year olds in Hackney and the City who are overweight. The
programme offers support to young people in achieving healthier
lifestyles through physical activity, healthier eating and
behaviour change, with an emphasis on having fun and building
self-esteem. Participants are offered a structured 10-week
programme of sessions covering a wide variety of health-related
topics and parents/carers and other family members are also
supported and encouraged to participate. GPs, school nurses,
teachers and other professionals may refer participants into the
scheme and potential clients or their families may also
self-refer.

Fit for Health
Hackney council, in partnership with NHS City & Hackney, are
delivering a physical activity scheme to help sufferers of a stroke
or transient ischaemic attack (TIA).

CHYPS Plus
CHYPS Plus offers services for young people aged between 11 and
19 years old at The House behind Lower Clapton Health Centre. They
complement existing general medical services offering adolescent
services and resources. The environment is confidential, and the
team is specially trained in providing youth centred care.

CHYPS Plus offers a holistic and flexible service that is
available at times most convenient to young people. A doctor and
nurse are on site as well as development workers, counsellors and
dieticians. Obesity is one of the five focus areas for the service
along with sexual health, chronic disease management, smoking and
mental health.
